ATHLETIC CLEARANCE PACKETS/PHYSICALS
*Packets submitted after May 31st, 2026 will be processed after the Athletics Office re-opens in July. Please make sure you are selecting the 26-27 school year when submitting your packet.
Important:
- All athletes must complete and submit a new Athletic Clearance Packet for the 2022-23 school year in order to be cleared to participate in any sport.
- Packets are required for all athletes (new and returning).
- Packets must be submitted before they can participate in tryouts, practice, or summer workouts.
- Students who do not submit their athletic packet (refer to "first" and "then" below) will not be able to participate in any athletics regardless of the reason. Coaches will turn away athletes until they receive a yellow receipt for that specific sport.
- Packets will be accepted starting May 13, 2026.
- Physicals are good for one calendar year. we recommend scheduling your physicals for the middle to end of May so it will be valid the entire school year and will cover all sports seasons (Fall, Winter, and Spring).
- We do not clear an athlete for a sport unless their physical will be valid for their entire season. If the physical expires during a season will result, the athlete will not be cleared for that sport until a new physical has been submitted.
- The Athletics Office will be closed June 16th - July 15th 2026
Online Athletic Packets (all steps required):

- Print and have your health care provider complete: Athletic Physical Form .
- Doctor must sign, date, and stamp this form before uploading into home campus.
- You will receive a confirmation email once application is "Clear".
